Council #10 Brochure – This brochure is an overview of Council #10.

A Retirement Benefit Review Brochure – This brochure provides an overview of pension, health, dental, life insurance, Social Security, and Medicare benefits available to retirees.

Dental Plan for Eligible Members Brochure – This brochure provides a summary of “closed plan” dental benefits available to eligible members through the Council #10 Dental Benefit Plan as provided by Dr. John D. Kernan, D.M.D.

Comparison of Closed Dental Plan & Freedom of Choice Dental Plan Benefits – This information sheet provides a comparison of the benefits in the “closed” and “Freedom of Choice” options available to eligible members through the Council #10 Dental Plan. Eligible members may switch plans only during the open enrollment periods (mid-December through late January).

"Freedom of Choice" Plan Dental Reimbursement Form" – This form must be completed by the member’s dentist and submitted to “Council #10 FOC Dental, c/o Dr. Kernan’s Office, 664 Cuthbert Blvd., Westmont Plaza, Westmont, NJ 08108” for partial reimbursement of payment in accordance with the FOC reimbursement schedule by those eligible members using the “Freedom of Choice” plan. Note: Those using the “closed” plan through Dr. Kernan’s office do not submit this form.

Dental Benefits for Retired Members – This information sheet explains the retiree dental plan. The accompanying form must be submitted to the Council #10 office at the time of retirement, along with payment, in order to participate in the plan.

New Jersey Paid Family Leave Insurance - The brochure provides information on the State program providing paid leave for the care of dependents.

Dental Benefits Complaint Form – This form should be completed and submitted to the Council #10 office by participants in the plan if they have a problem with dental service. A different complaint form is available at the Dental Office for submittal directly to that office. Members are encouraged to complete either the form at the Dental Office or this form whenever they have a problem. A response to all complaints will be provided.

Council #10 Death Benefit – This information sheet explains the Council #10 Death Benefit. The accompanying “Death Benefit Beneficiary Designation Form” must be completed, signed, and submitted to the Council #10 office in order for a member to be eligible for the benefit.

Your Rights Under the Federal & State Family & Medical Leave Acts – This information sheet provides basic information on the Federal and State Family & Medical Leave Acts. Members encountering family or medical problems requiring their absence from work should review the information provided in order to know their basic rights. Questions regarding the Federal & State Family & Medical Leave Acts should be directed to Council #10 representatives or the Council #10 office.

Weingarten Rights – This information sheet provides a summary of your rights to union representation at an investigatory interview where an employee has reasonable grounds to believe that the investigation may result in disciplinary action.

What is New Jersey Family Leave Insurance? - Questions and answers on the new paid leave insurance covering almost all workers in New Jersey.

Application for NJ Family Leave Insurance Benefits- The application

Council #10 PAC Application – This application must be submitted to your employer in order to allow payroll deductions to the Council #10 Political Action Committee. Members’ dues are not used for political purposes. The PAC is funded completely through separate contributions from members and acts to forward the interests of Council #10 and its members through the political process. Please consider donating $1 per pay period to the Council #10 PAC.
